Pre-Employment Application

Before you fill out the application, please take a look to see if you're qualified.

  • At least 23 years of age
  • Present a clean, professional appearance
  • Provide a complete and accurate application
  • Hold a Class A CDL
  • High school graduate or GED equivalent
  • No record of driver's license suspension or revocation for more than 30 days during the three month period prior to the date of the application (suspensions for non-moving violations are excluded)
  • No more than three minor moving violations in the past 36 months in any motor vehicle
  • No convictions in the last five years in any motor vehicle:
    a) of driving under the influence of alcohol or drugs
    b) refusing to take a test required by an implied consent or similar law
    c) of "open container" or similar statute
    d) of operating under suspension or revocation
    e) of leaving the scene of an accident or "hit and run"
    f) of speed exhibition, contest or drag race
    g) of assult with a motor vehicle
    h) of reckless driving or careless endangerment
  • No more than one preventable accident in the last 12 months
  • No preventable D.O.T recordable accident in the last five years
    (Recordable accident is defined as: An occurrence involving a commercial motor vehicle on a highway, in interstate or intrastate  commerce which results in: a) fatality b) bodily injury to a person who, as a result of the injury, immediately receives medical treatment away from the scene of the accident, or c) one or more vehicles incurring disabling damage, requiring the motor vehicle(s) to be transported away from the scene by a tow truck or other motor vehicle.)
  • No preventable accidents resultin in a fatality lifetime
  • No serious traffic violations while operating a CMV in the last three years
    (Serious traffic violations are: Excessive speed [15 mph or more over the posted limit], reckless driving, improper or erratic lane changes, following a vehicle to closely and offenses committed in a CMV in connection with fatal traffic accident.)
  • No past positive D.O.T or Non-D.O.T drug or alcohol test resulting from a test given by a previous or prospective employer

I hereby request and authorize Wooster Motorways Inc. and their agents or contractors that receive this application to cause to be conducted, at any time, an investigation of my background for employment purposes, which may include, but is not limited to, any information relating to my character, general reputation, personal characteristics, mode of living, criminal history, past work experience, educational background, alcohol or drug test results, or failure to submit to an alcohol or drug test, or any other information about me which may reflect upon my potential for employment gathered from any individual, organization, entity, agency, or other source which may have knowledge concerning any such items of information. As part of our consideration of your application, the DOT requires companies to investigate your employment background. As part of this investigation, they may obtain consumer reports about you from DAC Services. DAC is a consumer reporting agency. Any decision they make not to hire you based on information contained in your consumer report will be their decision alone. DAC does not make any decisions concerning your employment with these companies and will not know the specific reasons why they may decide not to hire you. In the event you are not hired based on information contained in your consumer report, the companies them selves will tell you. We will also advise you of your right to obtain a free copy of the consumer report from DAC and your right to dispute the accuracy or completeness of your report.Your consent for these companies to obtain the report from DAC is required. Although you have a right to withhold your consent, companies will not consider your application if you withhold your consent.
